Development of an Individual Learning Plan for a child with additional needs

Individual Learning Plan

This is an Individual Learning Plan that was developed for a child in my class, in collaboration between me, the Student Welfare Coordinator, and the child's parents. The plan incorporated strategies for both face-to-face and remote learning, and was informed by a student profile completed by her parents and additional information around literacy support strategies provided by an external support agency the child accessed outside of school. According to DIBELS testing (one of the indicators of success), the child improved their reading speed from 28 words correct per minute (WCPM) in July 2021, to 36 WCMP in October and 57 WCPM in December. This equates to an increase in WCPM between July and December of 103 percent.
